Purpose of Telehealth

Telehealth allows a licensed healthcare professional to evaluate and communicate with you using electronic technology when you and the healthcare professional are in different locations. Telehealth may include secure questionnaires, messaging, telephone, audio-video communication, electronic records, photographs, or other methods permitted by law and considered clinically appropriate.

Potential Benefits

Telehealth may improve access to care, reduce travel, allow efficient review of health information, support follow-up communication, and enable treatment when in-person care is not required.

Potential Risks and Limitations

Telehealth has limitations and risks. Information available remotely may be incomplete; a physical examination or diagnostic testing may be needed; technology or communication failures may delay care; electronic communications may be subject to privacy or security risks; and a healthcare professional may determine that telehealth is not clinically appropriate for your condition.

If a healthcare professional determines that remote care is insufficient or unsafe, you may be instructed to obtain in-person evaluation, laboratory testing, urgent care, or emergency care.

Independent Clinical Judgment

MDRx Labs LLC is not a medical practice and does not provide medical advice, diagnosis, or treatment. Clinical services are provided by licensed healthcare professionals who exercise independent professional judgment. MDRxLabs does not control or direct prescribing decisions.

Completing an intake, paying for a program, or requesting a specific medication does not guarantee a prescription. The treating healthcare professional may decline to prescribe, change a treatment plan, require additional information, discontinue treatment, or recommend in-person care.

Medication and Compounding

If medication is prescribed, the treating healthcare professional will determine the medication, dosage, instructions, and monitoring that are clinically appropriate. Where a compounded medication is prescribed and lawfully available, you understand that compounded drugs are not FDA-approved and are not reviewed by the FDA for safety, effectiveness, or quality before marketing.

Your Responsibilities

You agree to provide complete and accurate medical history, medication, allergy, pregnancy status where relevant, prior treatment, and other information requested by the healthcare professional. You agree to ask questions when you do not understand instructions and to promptly report adverse reactions, material changes in health, or new medications.

You must be physically located in a state where the treating healthcare professional is authorized to provide care at the time of the telehealth encounter. You agree to accurately report your physical location when requested.

Privacy and Medical Records

Telehealth records and health information are maintained in accordance with applicable privacy and medical-record laws. The healthcare provider's Notice of Privacy Practices explains how protected health information may be used or disclosed and describes your HIPAA rights where HIPAA applies.

Emergency Care

Telehealth through this program is not for emergencies. Call 911 or seek immediate in-person emergency care for a medical emergency, severe or rapidly worsening symptoms, or any condition requiring urgent evaluation.

You may withdraw consent to future telehealth care at any time by notifying the treating healthcare provider or contacting MDRxLabs support, subject to applicable law. Withdrawal does not affect care already provided or actions already taken in reliance on your prior consent and may limit your ability to continue participating in the online program.

By selecting "I Consent" or otherwise affirmatively accepting this Telehealth Informed Consent, you acknowledge that you have read and understand the information above, have had the opportunity to ask questions, understand the potential benefits, risks, and alternatives to telehealth, and voluntarily consent to receive healthcare services through telehealth.
